Fix mesher for models with high contrasts and fluid uppermost layers #48

Some fixes in the Mesher and Solver, that make it work with Venus-like models (planet plus dense atmosphere):

removed assumptions about fluid layer being between solid layers
more stable placing of coarsening layers (away from solflu boundaries)
receiver-free mode in Solver. In combination with Instaseis, receivers are not needed anyway.
automated testing of MESHER on all Model files that are in MESHER/Models directory
Add model files for Mercury and Venus
