Cancelling from build of master dark frame in bad-pixel-map creates multiple problems #316

Based on a report by user of 2.3.0: 

What steps will reproduce the problem?
1.  Run PHD2.exe outside the debugger
2.  Use a profile that has no bad-pixel map
3.  Click on Bad-pixel map menu item...
4.  Cancel when prompted to obtain the master dark frame
5.  Repeat this process - PHD2 will eventually bring up a dialog to "Refine 
bad-pixel map" even though no map exists.  Adjust the sliders and the app will 
abort. 
6.  Before doing step 5, note that user is also blocked from building a dark 
library

What is the expected output? What do you see instead?
