It seems that data URLs with base64-encoded images are not supported now. Please consider adding them one day =).

Could you please attach sample file that includes such image?

Sure. I was wrong about calling it EPUB3, it was EPUB2. Here it is.

It was created by Pandoc (also posted a bug to it to support separate formula images in EPUB).

Markdown source was:

"Кавычки"

$$ \sqrt{-1} = \pm i $$

Command line was:

pandoc -f markdown -t epub2 --smart --html-q-tags --webtex -o test.epub test.markdown

This file is also an example of <q>quotes</q> handling.
