When using --tv-quality fhd to download, for example, pid b080bz3t, get_iplayer reports it is downloading audio+video, but it only downloads audio as an mp4. For the next episode in the series, pid b0813szz, get_iplayer behaves as expected: downloading the audio then the video then muxing them into an mp4 container. This bug is not limited to this single pid; I have noticed a few instances where get_iplayer reports fhd is available, but only downloads audio.
