Go back to connect form on disconnect #173

As a developer, I might think that my users don't want to disconnect and not connect another wallet. They could just want to connect to a different wallet.

Therefore, I think it makes sense to give the option that on disconnect, the modal isn't fully closed but you go back to the modal where you can select wallets. This saves one extra click for such users.
