Improve agent performance #249

Various other fixes and tweaks as well. See commit comments. As part of manual testing, I confirmed that all the tests in https://github.com/land-of-apps/flowable-engine now pass and generate AppMaps.

The switch to Byte Buddy also resolves the instrumentation issue described in #214. How well Kotlin is actually supported still remains to be investigated.
