Create consistent entry-header classes #181

Currently there is little standardisation of .entry-header classes causing problems with headings in different contexts and locations.

Suggest making what we think of as "page-titles" into entry-header and then having entry-title and entry-meta.

When a entry-header has entry-meta immediately below it we can apply a modifier class of entry-header--has-meta to cancel the bottom margin.
