A cleaned up version of https://github.com/getsentry/sentry-auth-onelogin/pull/2

Supports configuration for SLO
Uses forms exposed in the sentry saml2 authentication module.

Requires https://github.com/getsentry/sentry/pull/6146

@mattrobenolt: Could I also get a final review on this?

It includes the Advanced form, which is maybe not something we need, I'm still trying to understand when we might need these options. But if we decide we don't need it we can easily remove it in a future PR.
