Empty page showing up when clicking the "Create jira button" #96

Here's what it looks like. Nothing shows up.

I hacked up templates/sentry_jira/create_jira_issue.html to duplicate a block of html that is letting you create the jira. If I do so the formatting is messed up, but I am able to create a JIRA.

Please note: My account does not have admin permissions.

This does not happen with the development version of the plugin that I did install manually. It would be good to cut a new release for folks who are on the latest sentry.
