Add an option to specify extra fields in settings #2

These changes allow to specify various extra fields in the plugin settings. It is useful if there are some required custom fields, or if the new issue should be assigned to someone right away, or perhaps to assign specific people as watchers, etc. Any feedback is welcome.

I think this could be more powerful, but its fairly complex to do dynamic forms right now in Sentry so this seems reasonable. Thanks!
