Request An Additional Meeting Page Not Functioning #27

Expected Behavior

There are links on each recent event page to the page Request an Additional Meeting, found under Meetings and events. This page is viewable in Wagtail and should take the user to a form submission.

Actual Behavior

Server returns an internal server error despite the page being viewable internally. e5a4f3294550d71257f36cb9d0601f33

This seems to be a general form template issue as the "Contact" page that uses the Form template returns a similar message: https://www7.ietf.org/contact/contact-ietf-secretariat/

I've passed this along to Torchbox via Codebase ticket for attention.

The form function is fixed but there is now a bug in the template style, adding bullets to form labels. TBX is investigating.
