Added Postfix, to allow to differentiate between Prod, Test, Sandbox, Dev, easily.

Added Proxy, to allow to add different Bastion Hosts per VPC / AWS Account.

I cherry picked this with commit https://github.com/gianlucaborello/aws-ssh-config/commit/3fcbe31c190b6e610bcbd6d7fa8203e0cc8c7633, the original branch seems to not exist anymore so I couldn't properly pull the commit.
