Handle exception if there's duplicate TitleID in titlekeys.txt #79

This commit handles exception if there's duplicate TitleID in titlekeys.txt causing error "An item with the same key has already been added." when converting to dictionary

Here's sample of duplicate TitleID (titlekeys.txt generated from the latest nutdb)
