Do you have the new or old Google Bar?
If you go to the Google Reader tab and the go to "Tools > JavaScript Console >
Errors" Do you see any errors? If so, please take a screenshot of them

It is still the old google bar. There is no error in the console.
"Hide Google bar" seems to work. Unchecking that and just checking "Hide
Header" seems to hide the Google bar, but not the header.

Hmm I'm guessing it's a Google Bar issue then. I tried to update the code
without breaking compatibility with the old Gbar but I don't have it anymore so
I couldn't really test.
You can force update to the new Google Bar with these instructions:

Seems like Google changed their mind about the Google Bar and are reverting
back to the old Gbar:

i have the same issue. hiding the header sometimes works (initial page load),
but sometimes it unhides itself, for example after a refresh (F5). if i check
the individual header items, it keeps the header items hidden, but leaves a big
white space where the header was.
i have the old google bar (tried your instructions to force new but to no
avail), no java console errors and am running:
Minimalist version - 0.5.20
Relevant core module - Reader
Chrome version - 19.0.1049.3 dev-m
Operating System - Windows 7
Language - English
Google Apps user? - No
Other extensions - Abine TACO 1.50, Adblock Plus (Beta) 1.2 *issue still
exists with other extensions disabled

hiding the header also seems to break the refresh 'r' shortcut key for me. it
threw a java error (attached). the same error is thrown if i use the goto all
'g a' command or click on the "View All Items" link when my list contains no
unread items.

I find that this happens to me occasionally, and about half the time a page
refresh fixes it, while the other half the time waiting a few hours and then
refreshing the pages fixes it (I assume due to changes made to Google's code
that broke it temporarily).
