I couldn't fill the replacement form #10456

Hi I couldn't fill in the replacement for the cards form, as I didn't receive the swags. is it possible that it can be done somehow?

I didn't even know this was a thing until just now. Haven't gotten any swag in the mail. Is the replacement form still available?

@ArchanGhosh You can't README.md you can only re-submit before March 1
