Update app.yml with missing default_events
README.md:
- Put all environment variables in one spot
- Fix grammar and formatting
deploy.md:
- State deployment environment variables at the top
- Update Helm instructions to use new OCI chart
- Update app permissions and events
Consolidate and update sample setting files
