Create the ticket checklist object #524

Mingle Card: 264 Create the object as described below. Most of these fields should be lookups just like they are in the existing job and ticket objects.

Create a validation on this object so that for the selected business segment, office, account and job type this ticket checklist will be unique.

It is OK for there to be one ticket checklist where all of these values are null
