Add survey link #41

@brianamarie @hectorsector @a-a-ron I'd like to add a survey to this course to gather some feedback from those who complete it. We're trying to understand the following:

  1. Do people feel like they can apply what we taught them in the course?
  2. Are they satisfied with the course (NPS)?
  3. What was their motivation for taking the course?
  4. What do they want to do next?

We are also asking them if they'd be interested in talking to us about their journey! At this point, we are only planning to keep this survey active for a few weeks so I'm keeping this PR revertible :wink:
