This PR:

Adds some misc improvements to project setup
Adds an experimental CachingAlleleTranslator that a) caches responses and b) adds a threaded method
Adds a new test case of BRAC1 see gnomad portal

Results:

Caching is working fine, although the cache strategy I selected is naive, perhaps use diskcache, see https://github.com/ohsu-comp-bio/vrs-python-testing/issues/8

Update

This commit seems to improve threading. All test pass using thread pool of 20.
