Program crashes when CSV file has lines with too few values #22

The parser needs to show an error message when a line in a CSV file doesn't 
have enough columns, 
and end up in a stable state.

It would also be useful to perhaps highlite the columns and rows with 
incomplete 
data during the import process.

An error message and failed imported is also received when a csv filename is 
all caps, (including the csv extension).
If lowercase is a requirement, an all caps occurance should end in a stable 
state instead of a crash. Although, perhaps all-caps can be accommodated. It is 
the default CSV export format from SQL Navigator.
