chore: adjusted contributing instructions with a note to make new issues #223

This probably should be a docs or chore or something also, since it's not exactly a feature of the mod. Can just be squashed and merged if necessary

I'm not sure if it's really necessary to be creating an issue for every fix. Especially since half of the completed fixes already don't have them, and there's the running list on the Nexus page.

It'll be a lot easier to track issues and fixes on the board, is all. There's not much we can do about past undocumented fixes, but we can at least do this for future additions.

The Nexus page (or rather the post on the nexus page) doesn't feature credits for who made the fixes or opened the issues, something which can be tracked via the board more easily. It also gives me less of an editing job when it comes to video production. All the information is in one place and is easily trackable.
