Fix regressions in cluster testing setup #1

The following problems prevent us from running clean cluster tests in EC2:
1. RMI doesn't bind to the correct adapter (now it will bind to internal 
adapter)
2. Cluster rolling upgrade version can't be null (now null = version "0")
3. Subsequent requests in the SIP LB are sometimes incorrectly classified as 
initial requests (now we use To-tag to determine subsequent)
4 . Nodes flip-flop out of availability (now we will timeout with more 
tolerance, same as the it used to be in the JSIP-HA side)
