Formatting stuff and text placement probably needs some work but it works otherwise.

Looks good to me. Has this been tested on all versions? Adding new addresses for every version is an error-prone process that has caused problems in the past, so I'd just like to confirm that all of the addresses are good.

Just tested them on all versions. All work fine.
