I want to define the workflow for issues and pull requests better.

For every change to the repository, there shall be an issue where the change is discussed.
Pull requests shall only be opened to implement the changes discussed in an existing issue. The discussion in a pull request shall only refer to the details of its implementation.

I will try to describe this in the contributor guidelines, and set up automation as much as possible to enforce this workflow.
