Fixes in javascript and added ir_to_xml header back in

IR2MSEXCEL.js :

• namespaced the functions so they won't live on window anymore. They're under the "excel_gpv" namespace now.
• Ensures that "$" is "apex.jQuery", avoiding problems with multiple jQuery libraries. (Fixes #14 )
• colWidths: checks if static id is passed in, throws an error if not
• download icon: didn't account for image directory as "/i/" was hardcoded, now uses the correct variable.
• Uses apex.util.htmlBuilder to create the html to be appended

IR_TO_MSEXCEL:

• changed the javascript calls to call the functions in the "excel_gpv" namespace

IR_TO_XML:

• The previous commit had the package header removed from the code, which caused the installation of packages to fail as a whole

install_all_packages:

• recreated this file since IR_TO_XML and IR_TO_MSEXCEL were changed

README:

• changed a few typos

Plugin sql:

• export of the plugin to account for changes in javascript file

I'm not sure that using non-documented js-function apex.util.htmlBuilder is good, but in util.js i have found /*
