Fixing lens plane bug #64

Attempts to fix bugs #62 and #63 @FabienNugier @ntessore

I don't think this solves everything, but this assertion in Lens::combinePlanes() was commented out instead of dealt with. Now it will increase zsource if a lens plane with a redshift exceeding it is put in.
