Implement some missing common CCs #78

CC7 (Volume): Should be tied to master volume. CC120 (All sound off): assigner_voiceDone, reimplement -1 = all voices. CC123 (All notes off): assigner_allKeysOff, add flag to apply no matter int/ext keyboard.
