Improve "add permission" and "edit permission" interfaces

The permissions display is looking so much better, but we're still using the old-style interface when adding and editing permissions (& conditions).

We should be able to click "edit" on a row in the permissions display, which should pop up an interface for editing that specific permission, including changing the conditions (or adding/removing them), swapping roles, etc. And then the "add" view just makes a new one. One idea we discussed was that we could show the single row in the modal as a kind of preview.

Here are some use cases, let me know if I'm missing any edge cases you want examples for:

• simple granting of permission to individual I want to give you, Arty, permission to delete comments on posts in a forum, so that you can help me moderate the forum
• simple granting of permission to role I want to have moderators help me delete comments, and I'll probably need more help than just you, so I create a "comment moderator" role, add you to it, and then give that role permission to delete comments
• granting permission with filter condition I want to give moderators the ability to delete comments, but only ones that contain certain key phrases like slurs. so I create the permission that gives delete comment permission to moderators, but I add a filter condition so it only applies to comments with key phrases in them
• granting permissions with multiple conditions I want to give moderators the ability to delete comments, and I want to give them more ability to do so if the comments have the key phrases, and less if they don't have the key phrases. So in addition to the above (role moderators has ability to delete comment if filter condition "has key phrase" is met) I also create the following: role moderators has ability to delete comment if filter condition is not met, and a second moderator approves the deletion
• granting permissions to different audiences I want to give anyone the ability to leave a comment if it is approved by a moderator, and "trusted members" the ability to leave a comment without any conditions/need for approval
