add life stage, physiological state, body part in as fields in csv download #22

Fields source_specimen_life_stage, source_specimen_physiological_state, source_specimen_body_part, target_specimen_life_stage, target_physiological_state and target_specimen_body_part have been added to the csv download files as columns. Not many data sources provide this information, so I expect many blank fields.

@jhammock - please let me know if there's any other fields that you'd like to have.
