consider creating a feedback form #82

To lower barrier to share feedback (e.g., incorrect data, ideas) suggest to create anonymous feedback form in addition to existing github issues and email channels.

@EMTuckerLab has created a neat feedback form on the "how-to" page. Perhaps this will help folks to reach out. Closing issue for now.
