Define decision making process within Global.Health in contribution guidelines #777

If people want to propose new fields/features/whatever they should know who to ask/who can make the final decision. We should outline the decision process for such features in our contribution docs.

I added a subheading for improvements and wrote out a short process. The decison making party here is @outbreakprepared

Moritz, are you the final decision person here or should be someone else (Graham?)
