Indicate cause of death if due to the identified pathogen of interest or another cause, where provided in the source documentation

Further discussion with G.h team. Cause of death is not captured on the WHO outbreak toolkit CRFs. For G.h to determine a cause of death from available data in sources could require considerable interpretation of the available data. Therefore this information will only be captured as a comment where explicitly indicated.

Recommendation: likely not required, can be confirmed from results of delphi survey.
