add unit drop down for 10.4a #353

issue here

added a drop down for unit options in 10.4a if 'other' is selected, an additional input appears to specify other unit

to test:

  1. go to section 10.
  2. add date at the top of the page
  3. go to 10.4, select 'Yes'
  4. go to 10.4a, test out unit selections, including other

note: if text is entered in 'other', and the drop down selection is switched, the 'other' text does not automatically clear. related to issue here . Need a clean solution for this - will put more thought into it this week
