Add Terms and Conditions Link/Checkbox #378

Description

Adds a 'terms and conditions' checkbox which must be checked to create an account.

Type of change

[x] New feature (non-breaking change which adds functionality)

Instructions

Go to signup page
Click on the terms and conditions link to confirm a PDF opens up in a new tab
Attempting to submit without checking box should not complete
After checking box, sign up form submission should complete
