I checked the history of this form, and it has never included any field for the user's email. If you would like to suggest having that field added and the ability to upload files, please use the suggestions site.
If you have questions about modifying this form, you can post on the forum.

Hi
When I create a ticket via the self-service helpdesk form (/front/hepdesk) it does not ask email address of the writer in order to set it as requester during the injection of the ticket in GLPI. The capability to attach a file would be useful too. How can I proceed to have at least a field that ask the writer to fill his email address?
