Icons not showing correctly in toolbar

On my work desktop the default icons (Lego brick?) always appear although I've 
configured alternatives.  I've tried putting the icon files in several 
locations but none work.  This may result from settings configured by local ICT 
as icons work properly for me on other computers.  If you have no immediate 
suggestions it may not be worth pursuing this as it's likely to be a rare 
issue.  [Thanks for a very useful utility.]

What steps will reproduce the problem?

Configured custom icon file but see default icon.

What version of the product are you using? On what operating system?
1.8.4.847 Windows 7 Enterprise 32-bit.
