Add context-team management routines #21

Resolve #9

Adds shmem_ctx_{get,set}_team and some introductory text relating contexts and teams -- the latter of which will probably need expansion.

This looks good to merge to get the basic API calls into the document. Incorporating feedback for error return codes or other issues can be added in another PR.
