[ ] * middle school or high school
[ ] other (with text field)
- [ ] for other: just text field rather than checkbox + text field
[x] make explicit - is field required or optional. E.g.
[x] last name, first name, email should be optional (only username required)

UIUC :
[x] 1. add (popup) checkboxes student, staff, faculty to UIUC checkbox, popup if it was checked)\
[x] 2. "What is your major/discipline?"

choices:

UIUC (if click, show fields student, staff, faculty; text fields exist already with fields "Major/discipline/unit"
community (if click, show field for 'what do you do? (e.g. "high school student" "artist" "engineer"')
