Provide a fully configured web project with ELMAH implemented #243

Provide a fully configured web project with ELMAH implemented, working pages 
that generate errors (typical and ELMAH-triggered) and instructions/scripts for 
configuring logging to the various data repositories and notifications. One or 
two web pages that generate errors (click this button to generate a divide by 
zero error, click this button to generate a file not found error, etc).

Having a fully configured web project with working pages that generate error 
logs and notifications would simplify knowledge transfer to new projects under 
development.
