Country and State/Province selection doesn't work when lifterlms_registration shortcode used on a page

Reproduction Steps

1. Add [lifterlms_registration] to a new page on a site with Sky Pilot (and possibly other themes)
2. Visit the page as a non-logged in user

Expected Behavior

Form allows the country and state/province to be selected

Actual Behavior

Country can be selected but the state/provinces are not loaded

• Include a concise description of what actually happens (but isn't supposed to)

This issue has be recreated:

• [ ] Locally
• [ ] On a staging site
• [ ] On a production website
• [ ] With only LifterLMS and a default theme
