Add Exception_Occurence to Ada.Exception package #3

because DSA stubs require it. Copy a-except.adb from

https://github.com/simonjwright/cortex-gnat-rts/

and add Read/Write streaming procedures. Copy a-except.ads from the Ada Reference Manual and fix copyright header.
