Add "repository" field and basic README to packages #78

For better NPM support we should fill out the repository field in the package.json so the link shows up in the NPM listing.

Along with that change we should add a basic README to all published packages so they don't appear empty on NPM. This could be as simple as including the name of the package and its purpose with plan on improving the READMEs in the future.
