<ref></ref> tags are not automatically added around the citation

What steps will reproduce the problem?
1. Enter new reference information into Proveit.
2. Click "insert into edit form".
3. <ref></ref> tags are missing from the citation.

What is the expected output? What do you see instead?

I expect to see <ref>citation</ref>. Instead, just the citation is inserted 
without the very important <ref></ref> tags.

When were you using ProveIt? On what browser (including version) and
operating system?

I don't understand the first question. Chrome 27.0.1453.94 m on Windows 7 64 
bit.

If page-specific, what article were you editing?

Not page-specific.

Optionally, what is your Wikipedia username?

I tested in Chromium 25.0.1364.160 (a little behind), and could not reproduce 
this.  I'm closing for now, but I will post a note to
