Clean up source code and enforce consistent naming. #41

We should enforce consistent code structure and naming whenever possible.  For 
example, any node that is provided only for cloning (dummy) should have that in 
the name.  Capitalization conventions should be consistent.  

As noted in issue 37, all methods should have javadocs.
