Configurable MusicBrainz server address #408

For users who run their own mirror of the MusicBrainz database, there should be 
a configuration option for the hostname of the MB server for queries. The rate 
limit should also be configurable for this situation.

(NB: Both parameters are already configurable in python-musicbrainz-ngs; we 
just need to expose them in .beetsconfig.)
