Loan Form templates often fail due to bad formatting #196

The current RTF loan form templates are error prone, and fragile, because they 
rely and bespoke mark up in RTF which can become damaged by errant formatting.

A better solution to to use a Open Document format (.docx) and MERGEFIELD 
fields.

Added support for OpenXML (.docx) Loan Form Templates. Templates should use 
MERGEFIELD fields with the same field specification as with the RTF loan 
templates
