Add pronunciation field / category checkboxes to New Review screen #224

When I add a review, I currently can't add album pronunciation or 
rotation/local status from the add review screen, which only has fields for 
review text, author name, label and year. If I want to add those other details, 
I have to submit the review, then go back and click to edit the information 
I've submitted. It would be great to have all of these on the same page when I 
first submit the review.
