Allow creation of multiple-choice questions #106

While it's great that GFormIt can create Google Forms based on questions (and 
answers) found in a Google Doc, the only types of questions it can process are 
single answer or freeform (no answer) questions. It's critical that GFormIt be 
able to create multiple choice questions.
