SAME TEST QUESTIONS BEING ASKED OVER AND OVER AGAIN #152

What steps will reproduce the problem?
1. Log in in as MENTOR 
2. Complete Module 1 Tutorial
3. Take test

What is the expected output? Different questions should be asked, each time
a user retakes the test. What do you see instead? Same questions over and
over again.

What version of the product are you using? On what operating system?

Please provide any additional information below.

We have entered question and answer into system as we received from question 
and answer excel sheet. It is better if any one can mention about duplicate 
question and answer then we can remove or please provide a fresh question and 
answer sheet.
