FIXED: Tooltips, IE frameborder, IE mouseover toolbar

This is a version I spent a few hours taking apart an editing to fix a few
of the issues that bugged me:

- You can now set the tooltip (title) of an item through the TOOLBAR option.
- The annoying border active in IE8 and (I assume) IE7 has been fixed
- IE does not support the CSS property opacity, so I added IE support for
the opacity hover to the stylesheet (and adjusted the opacity values.. feel
free to adjust to your liking)
- Adjusted the background-position of html and removeFormat to fix an issue
with the icons under their icon being slightly visible.
- Added example.html file to show how the editor can be initialized + with
an external stylesheet (global.css).

Updated to apply tooltip to A tag and not LI tag (both work, but A tag is 
preferred)
